How I look after your information

Last updated: [date]

Who I am

[Your name], trading as [practice name], is the data controller for information you share through this website. You can contact me at [email address]. [If applicable: I am registered with the Information Commissioner’s Office, registration number [number].]

What I collect

When you use the contact form I receive your name, email address, how you’d like to meet, and anything you choose to write in your message. Please keep that message brief; we can talk about the details on our call.

Why I use it

Only to reply to you and, if you choose to go ahead, to arrange sessions. My lawful basis is your consent and, once we agree to work together, the contract between us. If we work together, I keep brief session notes, which I will explain at our first meeting.

Who else sees it

Nobody, except the services that make this possible: [form provider, e.g. Formspree], [email provider] and [video platform]. I don’t sell or share your information for marketing, and this site uses no tracking cookies.

How long I keep it

If we don’t go on to work together, I delete enquiry emails within [6 months]. Client records are kept for [X years] after our work ends, in line with my insurer’s and professional body’s requirements, then securely deleted.

Your rights

You can ask to see, correct or delete the information I hold about you, or object to how I use it. Email me and I’ll respond within one month. If you’re unhappy with how I’ve handled your data, you can complain to the Information Commissioner’s Office (ico.org.uk).
